Eligibility

Variable Hour Employees (Average 30 or more hours per week):

* A six (6) month measurement period will be used to determine if a variable hour employee averages thirty (30) hours per week or more.
* If an average of thirty (30) hours per week or more is met, then an employee will be eligible for insurance benefits on the 1st of the following month.
* Hours worked are based on all of the pay dates occurring in the previous six (6) calendar months. Calculation of the rolling six (6) month average is made at the beginning of each month for the previous six (6) months.

Full‐Time Hourly Employees (expected to work 30 or more hours per week, every week, at time of hire):

* Hours worked will be reviewed after two (2) months from date of hire to determine if the expectation of thirty (30) or more hours per week has been met and is expected to continue.
* If thirty (30) or more hours per week has been met and is expected to continue, then the employee will be eligible for insurance benefits beginning the 1st of the following month.
* If thirty (30) or more hours per week has not been met or is not expected to continue, the employee will follow the eligibility requirements of Variable Hour Employees.

Salaried Employees and Managers:

* Eligible for insurance enrollment upon date of hire or promotion.
* Enrollment is effective on the 1st of the month following date of hire or promotion. In the event of a promotion and the employee is already enrolled, the contribution percentage will change on the 1st of the month following the date of promotion.

Enrollment Process for All Eligible Employees

* Restaurants are notified each month of any employees who based on meeting the eligibility requirements are newly eligible for insurance enrollment effective the 1st of the following month. The restaurant will provide the newly eligible employee with a packet that includes all benefit summaries and necessary forms.
* All newly eligible employees must complete the Benefit Selection/Compensation Reduction Form to either enroll in benefits or waive coverage.
* Enrollment is effective on the 1st of the month following notification of eligibility if the employee elects coverage by completing and turning in the Benefit Selection/Compensation Reduction form within the specified time.
* Any eligible employee who waives coverage will not be eligible again until open enrollment the following March, unless he/she experiences a qualifying event (i.e. loss of other coverage through no fault of his/her own, marriage, birth or adoption of a child).
